Our aim is to make providing feedback as simple as possible for you. We have therefore listed a number of ways in which you can share your thoughts with us.
You can provide feedback whilst at the SARC, by speaking to a member of our team.
You can provide feedback when we do our follow-up call to check how you are doing and that you have been able to access any services you have been referred into.
You can provide feedback via telephone or email at any point. Contact details can be found on our SARC and MSAS websites.
You can provide feedback by completing our feedback questionnaire:
a) A SARC staff member who you did not meet at the SARC, would be happy to read the questions to you, capture your verbal answers and complete the form for you. Once completed, we will read your answers back to you, to make sure you are happy with them.
b) A hard copy of this questionnaire is available for you at the SARC, and can be completed during your visit or taken away with a pre-paid envelope and returned anonymously to us.
c) An online version of this questionnaire can be found on the MSAS website (by clicking on the button on the ‘YOUR VOICE’ page: https://www.sarc-msas.co.uk/your-voice. This will take you to a Google form and your submitted results will be sent to the MSAS team.
Once completed please email to email@example.com or you can return a printed copy to the SARC that you visited.
e) On request, we would be happy to send you an email attachment of this questionnaire.
